Insurances accepted: Aetna, Cigna, Multiplan, and United. Please fill out to appropriate insurance verification form before scheduling which can be found here.

Credit card info is collected at time of booking, but not charged until 24hrs after appointment.
A travel fee of $30 applies to anyone over 20 minutes away from 21218 zipcode.
